Yes—endometriosis surgery can affect fertility in both positive and negative ways, and the impact depends heavily on where the disease is and what needs to be done in the operating room. When endometriosis is distorting anatomy, causing adhesions, or affecting the fallopian tubes and ovaries, meticulous excision can help restore function and improve the chances of conception by removing disease while preserving healthy reproductive tissue.


At the same time, fertility can be impacted if surgery involves the ovaries (for example, treating endometriomas), because removing cysts or using too much heat can reduce ovarian reserve. In some cases, severely damaged fallopian tubes may be removed to lower ectopic pregnancy risk and support more durable implantation with assisted reproduction—an example of how “fertility-friendly” surgery isn’t always the same as “keep every structure at all costs.”
