Can endometriosis surgery affect fertility?

Topic:Fertility
Affected areas:pelvicabdominal

Yes—endometriosis surgery can affect fertility in both positive and negative ways, and the impact depends heavily on where the disease is and what needs to be done in the operating room. When endometriosis is distorting anatomy, causing adhesions, or affecting the fallopian tubes and ovaries, meticulous excision can help restore function and improve the chances of conception by removing disease while preserving healthy reproductive tissue.

At the same time, fertility can be impacted if surgery involves the ovaries (for example, treating endometriomas), because removing cysts or using too much heat can reduce ovarian reserve. In some cases, severely damaged fallopian tubes may be removed to lower ectopic pregnancy risk and support more durable implantation with assisted reproduction—an example of how “fertility-friendly” surgery isn’t always the same as “keep every structure at all costs.”

Because there isn’t one standard endometriosis operation, we plan surgery around your goals—pain relief, pregnancy, or both—using precise techniques designed to minimize tissue trauma and maximize completeness of disease removal. If fertility is part of your plan, explore our educational resources and reach out to schedule a consultation so we can talk through your anatomy, imaging, and the surgical strategy most aligned with your timeline.
