The short answer
IVF is often favored for moderate to severe adenomyosis or when pregnancy hasn’t occurred despite well-timed attempts. But it isn’t automatically the best first step: younger people with mild or focal adenomyosis may still conceive with IUI or timed intercourse.

In these situations, IVF can be paired with a frozen embryo transfer strategy. This gives us more control over preparing the uterine environment before transfer than approaches that rely on the cycle’s natural timing.
That said, IVF isn’t automatically the best first step for everyone. Younger people with mild or focal adenomyosis may still conceive with timed intercourse or IUI, depending on overall fertility factors. The right plan is highly individualized and usually depends on your age, symptom severity, and what we see on imaging, such as ultrasound or MRI.
