Should I use GnRH meds after endometriosis excision surgery?

It depends on your goals and what was found at surgery. GnRH medications (like Lupron or oral elagolix/Orlissa) can reduce estrogen and sometimes quiet symptoms, but they don’t “heal” endometriosis biology—they mainly suppress activity while you’re on them, and symptoms often return after stopping. Because estrogen is important for bone, brain, and cardiovascular health, deep suppression isn’t a long-term strategy for most people.


After expert excision, our focus is on durable relief and protecting function, so we individualize whether any post-op suppression makes sense based on factors like residual disease risk, ovarian/endometrioma history, adenomyosis, symptom pattern, and fertility timeline. Some patients feel best with no GnRH therapy after a complete excision; others may use short-term suppression as one part of a longer plan, especially if symptoms persist or recurrence risk is higher.
