Yes—adenomyosis can be missed on imaging, especially when it’s mild, patchy, or early in its course. Ultrasound is also more operator-dependent, so subtle findings may be under-called or interpreted differently between readers.
A normal or “borderline” ultrasound or MRI doesn’t rule adenomyosis out if your symptoms and exam still fit. In our practice, we interpret imaging in the context of your full clinical picture and may recommend a repeat study or a different imaging approach when suspicion remains high, then review treatment options based on your goals and symptom severity.
