Can endometriosis cause blood in urine during periods?
Yes—endometriosis can sometimes cause blood in the urine (hematuria), especially when endometriosis involves the bladder wall (often called bladder endometriosis, a form of deep endometriosis). When it’s cycle-linked—showing up right before or during your period—that pattern can be a meaningful clue. That said, hematuria isn’t required for bladder endometriosis to be present, and many people with bladder involvement never see visible blood.
Because blood in the urine can also come from other conditions (including infection, stones, or a bladder growth), we don’t treat it as “automatically endometriosis,” even if your other symptoms fit. Our approach is to map the full symptom pattern and use targeted testing—often urinalysis plus pelvic imaging such as ultrasound and/or MRI—to look specifically at the bladder and nearby structures and to plan next steps.
