Do bowel endometriosis symptoms require a colonoscopy?
Not usually. Most bowel endometriosis lives on the outside of the bowel or within the bowel wall (most often the rectum and sigmoid), so a colonoscopy can look completely normal even when symptoms are significant. Colonoscopy is better at evaluating problems on the inside lining of the colon, which is why it’s not a reliable test to confirm—or rule out—bowel endometriosis.
That said, a colonoscopy can be part of the workup when your symptoms raise concern for a different or coexisting condition (for example inflammatory bowel disease), or when there are “red-flag” bowel symptoms that need a direct look at the colon. In our evaluation process, we start with your full symptom story and cycle pattern, then use targeted imaging (often expertly interpreted ultrasound and/or MRI) to map suspected deep disease and plan next steps.
