Hysterectomy for adenomyosis: keep ovaries or remove?

In most adenomyosis hysterectomies, the uterus is the source of the adenomyosis symptoms—so removing the uterus is the definitive step, while keeping the ovaries often makes sense because they continue to support bone, heart, brain, and sexual health. Removing ovaries triggers an immediate surgical menopause, which can be a major tradeoff if your ovaries are healthy and you’re not at elevated risk for ovarian cancer. For many patients, the goal is durable relief from heavy bleeding and cramping without creating new hormone-related problems.


The main reason this decision gets more complex is overlap with endometriosis, which is common in patients with adenomyosis. A hysterectomy does not treat endometriosis outside the uterus, and persistent symptoms after surgery can happen if endometriosis lesions aren’t addressed at the same time.
