Yes—ablation can be one reason pain returns after endometriosis surgery, especially when lesions are deeper than they look on the surface. Ablation works by destroying the top of a lesion, but it may leave deeper disease, surrounding fibrosis, or nerve-irritating inflammation behind, which can continue generating symptoms. If your operative report mentions fulguration, cautery, laser, or “burning” lesions, that often signals an ablation-heavy approach.
That said, pain coming back doesn’t automatically mean ablation was the only issue. Pain can recur when disease persists in hard-to-reach areas (bowel, bladder, ureters, diaphragm), when adhesions/scar tissue become a new pain driver, or when adenomyosis or pelvic floor dysfunction is also present.
