Yes—excision surgery can help ovulation pain when the pain is being driven by endometriosis affecting the ovaries, pelvic ligaments, or deeper pelvic tissues, especially when adhesions or ovarian endometriomas are involved. By removing endometriosis and freeing scar tissue, we can often reduce the mechanical pulling and inflammation that make mid-cycle pain so intense for many patients.


That said, results vary based on where the disease is located, how completely it can be excised, and whether other contributors are present, such as adenomyosis, pelvic floor muscle spasm, or nervous system pain sensitization.
