Can endometriosis pain continue after excision surgery?

Yes—endometriosis can still cause pain even after lesions are removed, and it doesn’t mean your experience is “in your head” or that surgery was pointless. Endometriosis pain is multifaceted: some pain comes from the lesions themselves, but some comes from how surrounding nerves and tissues have been affected over time. When pain has been present for months or years, the nervous system can become sensitized, so normal sensations may be perceived as painful and pain can persist even after the original driver is treated.


Persistent or returning pain can also be related to more local pelvic factors after surgery, such as scar tissue/adhesions, residual disease in difficult-to-see or difficult-to-access areas, or recurrence over time. In many patients, overlapping contributors—like pelvic floor and myofascial pain, bladder/bowel pain generators, or other chronic pain conditions—can keep symptoms active even when excision was technically successful.
