When is post-op bleeding after laparoscopy not normal?

Light spotting or a small amount of vaginal bleeding can be normal after laparoscopy, especially if a uterine manipulator was used, your cervix was dilated, or you had work done on the uterus/ovaries. It’s also common to have some “old blood” discharge for a few days, and your next period may come earlier or later than expected. That said, bleeding should generally trend lighter—not progressively heavier.


Post-op bleeding is not considered normal if you’re soaking through a pad in an hour, passing large clots, having bleeding that rapidly worsens after initially improving, or if bleeding is accompanied by red-flag symptoms like faintness, shortness of breath, chest pain, fever, worsening one-sided pelvic pain, or a foul-smelling discharge. Ongoing heavy bleeding can signal a complication such as a cervical/uterine injury, infection, or (more commonly) a hormonal or cycle-related issue that still deserves prompt triage.
