Why is my laparoscopy negative but I still have pelvic pain?

A “negative” laparoscopy can be confusing—but it doesn’t mean your pain isn’t real, and it doesn’t always rule out endometriosis. Sometimes endometriosis is missed because lesions can be subtle, hidden (for example, behind organs or deep in tissue), or not fully recognized without an excision-focused approach and careful pathology. In other cases, the pelvis may look “normal,” yet inflammation and irritated nerves can still drive ongoing pain, especially if your nervous system has become more sensitized over time.


Another common reason is that endometriosis may not be the only contributor. Pelvic floor dysfunction, bladder or bowel sensitization, adhesions/fibrosis, adenomyosis, fibroids, vascular causes of pelvic pain, hernias, and even conditions like small fiber neuropathy or central sensitization can mimic or amplify endometriosis-type symptoms. Our approach is to start by listening closely to your symptom pattern—what triggers pain (bleeding, ovulation, bowel movements, bladder filling, sex, sitting, exercise), where it lives, and what it feels like—then pair that story with a targeted exam and the right imaging or testing.
