Will endometriosis show up on MRI?

Sometimes—but a normal MRI does not rule out endometriosis. MRI is best at picking up certain patterns, such as ovarian endometriomas (“chocolate cysts”), deep infiltrating disease (for example involving the bowel, bladder, or uterosacral ligaments), and related conditions like adenomyosis or pelvic masses. Superficial peritoneal endometriosis and small implants often don’t show up, even when symptoms are significant.


When we use MRI, we think of it as a tool for mapping and surgical planning, not a yes/no test. The most accurate diagnosis still comes from minimally invasive surgery with direct visualization and biopsy, but imaging—when expertly interpreted—can help identify suspected disease and clarify what else might be contributing to your symptoms.
