Diagnosis
Will endometriosis show up on ultrasound?
The short answer
Sometimes, but a normal ultrasound does not rule out endometriosis. Ultrasound is best at detecting endometriosis cysts in the ovaries and, in skilled hands, signs of deep disease or scarring; many superficial lesions are too small or subtle to be seen on standard imaging.
Sometimes, but a normal ultrasound does not rule out endometriosis. Ultrasound is best at detecting certain signs of endometriosis, especially ovarian endometriomas (endometriosis cysts) and, in skilled hands, patterns that can suggest deep infiltrating disease, scarring, or adhesions. Many endometriosis lesions, especially superficial peritoneal implants, are too small or subtle to be seen on standard imaging.
When we review imaging, we focus on what ultrasound can realistically tell us and what it cannot, interpreting it in the context of your full symptom history and exam. If your symptoms strongly fit endometriosis but the ultrasound is normal, that’s a common scenario and still warrants a thoughtful, endometriosis-informed evaluation. In some cases, additional imaging, like an expertly interpreted MRI, can add useful detail. For many patients, minimally invasive excision surgery remains the most definitive way to diagnose and treat endometriosis at the same time. Ultrasound and Your Symptoms
Ultrasounds can detect some endometriosis signs, but a normal result doesn't rule it out. Our specialists interpret imaging alongside your symptoms to guide accurate diagnosis and care.
Questions people ask next
All diagnosis questionsHow can I get diagnosed with endometriosis if my ultrasound is normal?
A normal ultrasound doesn’t rule out endometriosis; evaluation includes a detailed symptom history, targeted pelvic exam and, when appropriate, expert imaging. If surgery becomes part of the plan, testing removed tissue offers the most definitive confirmation.
How accurate is ultrasound for detecting endometriosis?
Ultrasound is very good at finding ovarian endometriosis cysts and can detect many cases of deep endometriosis when performed by someone with endometriosis imaging expertise. It rarely shows superficial endometriosis, so a normal scan does not rule out the condition.
Standard pelvic ultrasound vs endometriosis-focused scan: what’s different?
A standard pelvic ultrasound surveys the uterus and ovaries, while an endometriosis-focused scan uses targeted imaging and checks how pelvic organs move against each other. A standard scan may miss deep disease or scarring; a focused scan can help with surgical planning.
Is laparoscopy the only way to confirm endometriosis?
Yes. At this time, the only definitive way to confirm endometriosis is laparoscopy, with direct visualization and biopsy of suspected lesions. Imaging and exams can strongly suggest it, but can’t reliably rule it in or out in every case.
