Yes—coughing up blood that predictably happens just before or during your period can be a sign of thoracic endometriosis syndrome (TES), a rare form of endometriosis that involves the diaphragm, chest cavity, or (more rarely) the lungs. People may also notice cyclical chest pain (often one-sided), shortness of breath, shoulder pain, or episodes that keep getting labeled as “infection” but return with the next cycle. The key clue is the pattern: symptoms that track your menstrual timing are not typical for routine respiratory illnesses.


At the same time, coughing blood is not something we assume is endometriosis until we’ve carefully ruled out other causes and understood the full context of your symptoms. Our team focuses on a whole-picture evaluation—your flare timing, pelvic and non-pelvic symptoms, prior imaging and treatments—and we often coordinate the right imaging and specialty input when chest involvement is possible. If the story fits TES, diagnosis and treatment may involve targeted hormonal suppression for symptom control and/or planned minimally invasive surgery with the appropriate surgical team, sometimes including thoracic collaboration.
