Does painful urination always mean bladder endometriosis?
Not necessarily. Painful urination can happen with bladder endometriosis, but it can also be caused by other common issues like pelvic floor muscle spasm, interstitial cystitis/bladder pain syndrome, urinary tract infections, kidney stones, or vulvovaginal irritation.
The pattern of symptoms is often the biggest clue—especially whether pain predictably flares around your period and whether you also have pelvic pain, urinary frequency/urgency, or pain with bladder filling. A careful history and pelvic exam can help narrow the cause, and in some cases imaging may add useful information, although bladder endometriosis isn’t always visible on scans.
