How is painful urination evaluated with suspected endometriosis?

When painful urination may be related to endometriosis or adenomyosis, we start by mapping your symptoms in detail—especially whether burning, urgency, or bladder pressure flares predictably with your cycle, and whether there’s pelvic pain, painful sex, bowel symptoms, or heavy bleeding alongside it. We’ll also review prior urine tests, antibiotics, and any history of recurrent “UTIs” that don’t fully match culture results. This history helps us separate infection and irritation from symptoms that can come from endometriosis affecting tissues around the bladder.


A typical evaluation includes a urinalysis and urine culture to check for infection or blood in the urine, and a pelvic exam when appropriate to assess pelvic floor tenderness and other pain generators. Pelvic ultrasound and sometimes MRI can be useful to look for endometriomas, deep disease, or signs of adenomyosis, but normal imaging does not rule out endometriosis. If your pattern and exam raise concern, our team will discuss the next steps for confirming the cause and building a treatment plan—including when surgical evaluation may be appropriate—so you’re not left cycling through the same symptoms without answers.
