How can I tell if my period cramps are abnormal?

Period cramps may be more than “normal” when pain starts before bleeding, progressively worsens over months or years, or doesn’t meaningfully improve with common measures like NSAIDs and heat. A key red flag is impact: if cramps regularly disrupt school, work, sleep, or daily activities, that’s worth a deeper look rather than something you should have to power through.


We also pay attention to the pattern of bleeding and the quality of the pain. Heavy or prolonged bleeding with clots, along with a heavy, pressure-like uterine ache (sometimes described as a “boggy” or enlarged-feeling uterus), can be consistent with adenomyosis—especially when it occurs alongside ongoing pelvic pain.
