How do endometriosis stages affect fertility and treatment choices?

Endometriosis stage can influence fertility, but it doesn’t predict it perfectly. More advanced disease (often stages III–IV) is more likely to involve adhesions and ovarian endometriomas that can distort pelvic anatomy and interfere with tubal function and egg pickup. That said, some people with early-stage disease struggle to conceive, while others with advanced disease conceive without difficulty.


Treatment planning is individualized and usually considers your age, symptoms, ultrasound/MRI findings, prior surgeries, how long you’ve been trying, and whether there’s concern for tubal or ovarian involvement. In some cases, excision surgery is used to remove disease and improve anatomy; in others, it may make more sense to proceed directly to fertility treatment or to combine approaches.
