How do I tell endometriosis from IBS, IC, or pelvic floor dysfunction?

Endometriosis can overlap with bowel, bladder, and pelvic muscle symptoms, but a key clue is timing: many patients notice pain and other symptoms that flare predictably around their period, ovulation, or with deep penetrative sex. Bowel symptoms from endometriosis may feel like “IBS,” yet they often worsen cyclically and may come with pelvic pain, painful periods, or pain with bowel movements specifically during certain parts of the cycle.


IBS is more likely to track with meals, stress, and stool pattern changes (diarrhea, constipation, or both) rather than a monthly pattern. Interstitial cystitis/bladder pain syndrome commonly causes urinary urgency, frequency, and burning or pressure with repeatedly negative urine cultures, while pelvic floor dysfunction tends to involve muscle spasm, pain with sitting, and significant tenderness during pelvic exams.
