IBS often causes bloating, cramping, and constipation and/or diarrhea that can fluctuate over time, and it typically doesn’t come with red-flag signs like rectal bleeding, persistent fevers, or unintended weight loss. Symptoms may track with stress or certain foods, and the overall pattern is often variable. In contrast, IBD is more likely when there’s blood in the stool, nighttime diarrhea, ongoing fatigue, anemia, fevers, or weight loss—especially when symptoms feel progressive rather than episodic.
Endometriosis can mimic both because it can irritate or involve the bowel and surrounding pelvic tissues. Clues we look for include pain with bowel movements, deep pelvic pain, and bowel changes that are clearly cyclical—worse in the days leading up to or during your period, sometimes along with painful sex or period pain that isn’t well controlled.
