UTI or endometriosis bladder irritation: how can I tell?

A true UTI is an infection, so it typically shows bacteria on a urine test (especially a urine culture) and often improves with the right antibiotic. Bladder symptoms related to endometriosis or pelvic pain conditions are more likely to flare in a cyclical pattern—often around your period—while urine cultures stay negative and antibiotics don’t bring lasting relief.


Because the symptoms can feel very similar, we generally recommend confirming whether infection is present before starting treatment, so an infection isn’t missed and unnecessary antibiotics are avoided. If cultures are repeatedly negative, our team can help evaluate other common causes of “UTI-like” symptoms, including bladder pain syndromes and pelvic floor muscle irritation, and discuss next steps based on your full symptom pattern and history.
