Is surgery always needed for deep pain with sex?

Not always. Deep pain with sex can have multiple drivers, and surgery is typically considered when symptoms are persistent or severe, when the pattern of pain suggests deeper disease such as endometriosis, or when hormonal/medical therapy hasn’t helped or isn’t a good fit for you.


When endometriosis is a likely cause, excision surgery can be an important option because it aims to remove disease and free adhesions rather than simply suppress symptoms. Many patients do best with a combined plan that also addresses pelvic floor muscle guarding and pain sensitization before and after any procedure.
