What does deep pain during sex usually indicate?

Deep pain with sex (deep dyspareunia) often points to pain coming from deeper pelvic structures rather than the vaginal opening. In our experience, it can be associated with endometriosis in areas like the uterosacral ligaments, the cul-de-sac behind the uterus, or the tissue between the vagina and rectum, and it may also involve tight, reactive pelvic floor muscles.


A focused evaluation helps us sort out whether the pain is driven by endometriosis, pelvic floor spasm, adenomyosis, or other pelvic conditions that can mimic each other. We typically combine a targeted pelvic exam with appropriate imaging such as a high-quality ultrasound or MRI when it’s useful, then build a plan around what we find.
