Why does sex still hurt after endometriosis treatment?

Pain with sex can persist after endometriosis treatment because not all pain is driven by visible lesions alone. Even when surgery or medical therapy reduces disease, the pelvic floor muscles may stay in a protective, overactive “guarding” pattern, which can cause superficial burning, deep aching, or a tight/blocked feeling with penetration. Ongoing nerve sensitization—and in some people, central sensitization—can also make normal touch or pressure register as pain, especially if sex was painful for months or years before treatment.


Another common factor is recovery timing and tissue irritability after treatment, particularly after more extensive deep endometriosis surgery; healing can take longer than expected, and arousal, lubrication, and comfort don’t always rebound on the same timeline as surgical healing. That doesn’t automatically mean the endometriosis is “back,” but it does mean we should look at the full pain picture, including pelvic floor dysfunction and overlapping bladder/bowel or musculoskeletal contributors.


In our practice, persistent dyspareunia is a signal to reassess what type of pain you’re having (deep vs superficial, positional, entry pain vs thrusting pain) and match it to the right next step—often including pelvic floor therapy focused on relaxation, coordination, scar/connective tissue mobility, and calming pain pathways.
