What is central sensitization in chronic pelvic pain?


Central sensitization means the brain and spinal cord become extra reactive to pain signals after the nervous system has been under stress from persistent pelvic pain for a long time. In this state, the “volume knob” on pain processing is turned up—so pain can feel more intense than expected, last longer, or be triggered by sensations that shouldn’t normally hurt.
In chronic pelvic pain (including pain from endometriosis or adenomyosis), central sensitization can show up as pain that persists even when a flare “should be over,” pain that spreads beyond the original area (pelvis plus low back, hips, or more generalized achiness), or flares that feel disproportionate to activity. It can also travel with fatigue, poor sleep, brain fog, and heightened sensitivity—because the nervous system is on high alert, not because the pain is imagined.
For many patients, the most helpful framing is that pelvic pain can have both peripheral drivers (like lesions, inflammation, adhesions, or muscle spasm) and central drivers (how the nervous system processes ongoing threat).
