Pelvic MRI is most helpful when we suspect deep endometriosis involving structures like the bowel, bladder, ureters, or pelvic nerves. It can help map disease patterns beyond what a routine exam can show. We also commonly use MRI when a high-quality ultrasound doesn’t fully explain symptoms or when imaging is needed to clarify what’s driving pain, bowel issues, or urinary issues.
MRI can be especially valuable for preoperative planning, because understanding the likely locations and depth of disease helps us plan the safest, most complete approach. It may also be considered when symptoms persist despite prior treatment and knowing the extent of disease could meaningfully change next steps. MRI and expert transvaginal ultrasound tend to have similarly high accuracy for deep infiltrating endometriosis and ovarian endometriomas. Both can miss superficial peritoneal endometriosis, so normal imaging does not necessarily rule out endometriosis.
Can MRI reliably detect deep infiltrating endometriosis?
MRI can be very helpful for suspecting deep infiltrating endometriosis, especially when read by a radiologist experienced in endometriosis, but it is not a guaranteed yes/no test. A normal MRI does not reliably rule it out, and even deep disease can be missed.
Why would I need both ultrasound and MRI?
Ultrasound and MRI often complement each other because they highlight different findings. Ultrasound can identify ovarian cysts, while MRI can help clarify the location and extent of disease, especially in deeper areas or when planning surgery.
Can imaging confirm endometriosis?
Imaging can be very helpful, but it usually can’t confirm endometriosis on its own. Ultrasound or MRI may strongly suggest disease, but a normal scan doesn’t rule it out because many small, superficial or microscopic lesions won’t show up on scans.
