Why do I get brown spotting before my period each month?

Brown spotting right before a period is usually old blood leaving the uterus slowly, which can look brown rather than bright red. Many people notice this as a normal “start-up” to their cycle, but when it happens every month—especially with worsening cramps, pelvic pain, pain with sex, heavy flow, or infertility—it can also be a clue that something more than a simple timing issue is going on.

In our experience, recurrent pre-period spotting can overlap with conditions that affect bleeding patterns and pelvic inflammation, including endometriosis, adenomyosis, hormone-ovulation patterns, cervical or uterine lining issues, and other look-alike or coexisting conditions. The key is the full pattern: when the spotting starts, how long it lasts, what your pain and bowel/bladder symptoms do around it, and whether there are mid-cycle flares or progressively heavier periods.

If this is a consistent monthly pattern for you, our team can help you map the symptoms and evaluate the broader picture using a careful history, exam, and—when appropriate—expertly interpreted ultrasound or MRI. From there, we can explain what the spotting likely represents in your specific case and what next steps make sense, including treatment pathways that fit your goals.
