Pain that worsens in certain positions often points to mechanical pressure or stretching of sensitive pelvic structures—such as the cervix, uterus, ovaries, or tissue behind the uterus. With deeper penetration or certain angles, those areas can be contacted more directly, which is why one position may feel tolerable while another triggers sharp or lingering pain.


When pain predictably flares at specific times in your cycle, it can reflect hormone-driven inflammation and changes in pelvic tissues, which is common with conditions like endometriosis or adenomyosis. Many patients notice symptoms intensify around ovulation and in the days leading up to a period, and pelvic floor muscle guarding can amplify discomfort—especially if your body is bracing in anticipation of pain.
