Diagnosis
Can imaging find the cause of my painful periods?
The short answer
Ultrasound and MRI may show causes of painful periods, such as adenomyosis, fibroids, ovarian endometriomas, or deeper endometriosis. But imaging can’t explain every cause, and a normal scan does not rule out endometriosis or other sources of pelvic pain.
Imaging can be very helpful in evaluating painful periods, but it can’t explain every cause. Ultrasound and MRI may show conditions such as adenomyosis, fibroids, ovarian endometriomas, or deeper forms of endometriosis when they’re present and visible.
Still, a normal scan does not rule out endometriosis or other sources of pelvic pain. Many patients have significant symptoms despite normal imaging. In our practice, the most useful approach is an expert evaluation that brings together your symptom pattern, a focused pelvic exam when appropriate, and targeted imaging to clarify what’s most likely driving your pain. If your periods are progressively worsening or not responding to typical treatments, reach out to schedule a consultation so our team can help plan the next steps.

Questions people ask next
All diagnosis questionsCan imaging tests diagnose endometriosis or adenomyosis?
Yes, MRI and high-resolution transvaginal ultrasound can help identify endometriosis or adenomyosis and guide treatment planning. A normal scan doesn’t rule out either condition, as smaller lesions and superficial endometriosis can be missed.
Why can imaging look normal when symptoms are severe?
Imaging can look normal despite severe symptoms because many pain-generating conditions don’t reliably show up on ultrasound, CT, or MRI. Some disease can be small, superficial, or hard to see, yet still irritate nerves and surrounding tissues.
What tests help explain severe pelvic pain with normal imaging?
A detailed symptom history and pain-focused exam are often most useful when imaging is normal. Depending on symptoms, testing may assess pelvic floor problems, nerve-related pain or vascular causes; if surgery is part of your plan, biopsy can confirm endometriosis.
