Why don’t hormones help my endometriosis pain?
Hormonal treatments can help some people with endometriosis, but they don’t “heal” the disease—and that mismatch is a common reason pain doesn’t improve. Birth control, progestins, and estrogen-lowering medications mainly work by dialing down cycle-driven inflammation and bleeding, which can reduce symptoms for a while. If your pain is being driven by factors hormones don’t fully address—like nerve growth, ongoing inflammation, scarring/adhesions, or immune dysfunction—you may feel little relief even when your periods are lighter or suppressed. And even when hormones help, the benefit is often modest or temporary, with symptoms frequently returning after stopping them.
Another reason hormones may not help is that endometriosis pain isn’t purely “hormonal.” Over time, repeated pain signals can sensitize the nervous system, so pain can persist outside of periods or continue even after the hormonal cycle is suppressed. Pain can also come from overlapping contributors that commonly travel with endometriosis, such as pelvic floor muscle dysfunction or bladder/bowel pain syndromes—areas where changing estrogen alone may not move the needle.
In our practice, we look at the whole pain picture: whether there’s likely active endometriosis needing precise excision, whether there are signs of deep disease affecting organs, and whether central sensitization or other pain generators are amplifying symptoms.
