Can pelvic floor PT help painful sex?
Yes—pelvic floor physical therapy can be very helpful for pain with sex (dyspareunia), especially when pelvic floor muscles have become overactive, shortened, or “guarded” after months or years of pelvic pain. In endometriosis and adenomyosis, pain isn’t always driven only by visible disease; muscle hypertonicity and nerve sensitization can keep intercourse painful even when imaging is normal or even after surgery.
In pelvic floor PT, the goal is often not strengthening (“more Kegels”), but restoring relaxation, lengthening, and coordination of the pelvic floor with breathing and movement. Therapy may also include gentle, consent-based manual work to reduce tissue restriction and calm pain signaling, along with strategies to retrain the nervous system so touch and penetration feel safer over time.
