IBS-C vs endometriosis constipation: how can I tell the difference?
IBS-C can cause constipation, bloating, and changes in stool form, but endometriosis-related constipation often follows a menstrual pattern—worsening before or during your period and improving afterward. Many patients also notice pelvic pain, painful periods, pain with sex, or pain with bowel movements (especially with straining or during a period), which can point toward endometriosis as part of the picture.
That said, IBS and endometriosis can coexist, so it isn’t always an either/or situation. In our practice, we focus on a detailed symptom timeline (including cycle tracking), a pelvic exam, and—when appropriate—imaging to look for clues like deep disease or signs of bowel involvement.
