What do endometriosis stages I–IV actually measure?

Endometriosis stages I–IV mainly describe the visible amount and distribution of disease and scar tissue (adhesions), based on what’s seen and scored at surgery—most often during laparoscopy. In other words, staging is an anatomic scoring system, not a direct measure of how much pain someone has or how much their symptoms affect daily life.


Because staging doesn’t reliably capture nerve involvement, deep organ infiltration, or how inflammatory a lesion is, someone can have “low-stage” disease and still have severe symptoms. For deeper disease patterns, additional mapping systems (such as ENZIAN) and high-quality, endometriosis-focused imaging can provide a more meaningful picture.
