When is pelvic pain an emergency after surgery?
Some pelvic discomfort and cramping can be expected after endometriosis or adenomyosis surgery, but pain becomes urgent when it’s severe, rapidly worsening, or feels out of proportion to what you were experiencing earlier in recovery—especially if it’s not improving with your prescribed plan. Pelvic pain that comes with heavy vaginal bleeding (soaking pads), fainting, new confusion, or the inability to keep fluids down is not something to “watch and wait” on.
We also consider it an emergency if pelvic pain is paired with fever or chills, increasing abdominal swelling or a hard/tender belly, new shortness of breath or chest pain, or one-sided leg swelling/pain—because those combinations can signal complications that need immediate evaluation. New urinary retention, burning with urination plus fever, or escalating rectal pain/inability to pass gas or stool can also be red flags after pelvic surgery, particularly when bladder, bowel, or ureters were part of the surgical field.
