When endometriosis is part of an infertility workup, the male-factor evaluation matters because outcomes are often very different when endometriosis is the only issue versus when there’s an additional factor. The single most important starting test is a properly performed semen analysis, since it gives a broad, actionable snapshot of sperm concentration, motility, and morphology. If the semen analysis is abnormal—or if you’ve had unexplained infertility despite “normal” basic results—repeat testing is often helpful because sperm parameters can vary over time.


From there, the next tests that tend to matter most are the ones that clarify why the semen analysis looks the way it does and what it means for your next step (timed intercourse, IUI, or IVF/ICSI). Depending on the pattern, this can include hormonal testing (like FSH, LH, total testosterone, and prolactin) and targeted genetic testing in more significant sperm-count issues.
