Why do I get stabbing pelvic pain after sex?
Stabbing pelvic pain after sex is often a form of dyspareunia, and it can happen when intercourse or orgasm triggers sensitive pelvic tissues, nerves, or muscles. In endometriosis (and sometimes adenomyosis), inflammation and nerve sensitization can make the pelvis “overreact” to normal stretching and uterine contractions. Scar tissue and adhesions can also pull on organs during deep penetration—sometimes involving the uterus, cervix, upper vagina, rectum, bladder, or pelvic sidewall—creating sharp, catching pain.
Another common driver is pelvic floor dysfunction: when pain has been present for a while, pelvic floor muscles can become overactive and protective, and the contractions that occur with orgasm may trigger a flare afterward.
