Does bowel endometriosis surgery improve fertility?

Sometimes—but it isn’t guaranteed. Removing bowel (colorectal) endometriosis can improve fertility for some people, especially when disease anatomy is distorting the pelvis, involving the tubes/ovaries, or creating severe adhesions that interfere with egg pickup and tubal function. In those situations, excision surgery may help restore more normal anatomy and reduce inflammatory burden, which can make natural conception more realistic and/or improve conditions for IVF.


When bowel endometriosis is present but pain and bowel symptoms are minimal, many patients choose to pursue fertility treatment (often IVF) first, because complex bowel surgery can add time, recovery, and meaningful surgical risk—and some people still need IVF after surgery anyway. The right sequence depends on your symptoms, age and ovarian reserve, prior surgeries, and whether adenomyosis may also be impacting implantation.
