Why does my vulva burn after sex if tests are normal?
Vulvar burning after sex with “normal” testing is common—and it doesn’t mean the pain isn’t real. Many infections and STIs can be ruled out while symptoms persist from friction/irritation, vaginal dryness, allergic or irritant reactions (lubricants, condoms, semen, soaps), or skin conditions that aren’t picked up on standard swabs. Burning can also be driven by nerve sensitization (sometimes called vulvodynia), where the pain system becomes more reactive even when the tissue looks normal.
In people with endometriosis or chronic pelvic pain, we also look closely at pelvic floor dysfunction. Overactive pelvic floor muscles can clamp or spasm with arousal, penetration, or orgasm, creating superficial burning at the vulva/vestibule and sometimes lingering pain afterward—especially when the nervous system has become sensitized over time.
If this is happening repeatedly, our team can help you map the exact pattern (entry pain vs deep pain, burning vs tearing, immediate vs delayed), examine the vulva/vestibule, and evaluate for pelvic floor and “overlapping neighbor” conditions that can mimic each other.
