Hip pain during periods: could it be endometriosis?
Yes—hip pain that predictably flares during your period can be endometriosis-related, even though it’s not the “classic” pelvic cramp people expect. Endometriosis can irritate pelvic tissues and nerves and refer pain into the hip, buttock, or upper thigh, and in some cases it can involve or compress nerves such as the sciatic or femoral nerve. Many patients describe pain that starts before bleeding begins and lingers after the period ends, or pain that radiates and feels burning, stabbing, or deep and aching.
That said, period-timed hip pain doesn’t automatically equal endometriosis, and we also look carefully for look-alike or coexisting drivers—like pelvic floor muscle spasm (including piriformis-type patterns), hip joint pathology, hernias, or vascular and spine-related causes—because treatment depends on the true source. In our evaluation, we focus on your full symptom pattern and exam findings, and when appropriate we use expertly interpreted ultrasound and/or MRI to look for suspected endometriosis and related pelvic conditions (while recognizing that imaging can miss disease).
