Can adenomyosis cause ovulation pain?

Yes. Even though adenomyosis is a condition within the uterine muscle, it can cause pelvic pain outside of your period, including mid-cycle pain that lines up with ovulation. Many patients describe this as cramping, deep pelvic pressure, or an aching sensation that feels uterine rather than sharp and one-sided.


Around ovulation, normal hormonal shifts can make the uterus more reactive and sensitive, which may amplify inflammation and cramping in someone with adenomyosis. It’s also common for adenomyosis to overlap with endometriosis, and that combination can make ovulation-time pain more frequent or more intense.
