Can surgery cause adhesions that worsen pain?

Yes—any surgery in the pelvis or abdomen can lead to adhesions (bands of scar tissue), and in some people adhesions can contribute to ongoing or worsening pain by restricting organ movement or tethering sensitive tissues. It’s also important to know that endometriosis itself can create significant scarring and a “frozen pelvis,” so pain after surgery isn’t always from new adhesions; it may reflect pre-existing disease-related fibrosis or remaining disease.


The risk of problematic adhesions depends heavily on the type of surgery and how it’s performed. Meticulous excision with gentle tissue handling, precise dissection, excellent visualization, and minimizing thermal injury are all strategies that can reduce unnecessary trauma—one reason our team favors robotic excision for complex and re-operative cases. If pain persists after a procedure, we also evaluate other common contributors like pelvic floor and myofascial pain, nerve-related pain, and central sensitization, since these can mimic “adhesion pain” even when healing is anatomically normal.
